Revised an info card with tear-off form

A “Connect Card” design was creating confusion in filling out the card, and deterring submission. It asked for personal details with little context for what would happen or who would see it. The event information was on the back of a form, forcing a decision to submit the form or keep events details (or throw it away!).

Research and testing

I reviewed survey responses, interviewed stakeholders, and tested content with audience users to check for clarity, ease of use, and security.

The result

The final card design received excellent feedback in the coming weeks. More people submitted connect cards or completed all the information, so that staff members could contact them.

Revised Connect Card

  • Top front includes event details, with notes on the reverse.
  • Tear-off form is dual-sided, with context about requesting private personal details.
